For your convenience, request routine appointments online. If you are ill and would like to be seen right away, please call our office at (623) 544-8400. We triage all calls, and we do our very best see patients with urgent issues the same day.
I need to renew a prescription
It is our practice to provide a sufficient amount of medication to our patients until they are due for a follow-up appointment. If you have run out of medication, it is likely that you will need to schedule an appointment.
Please contact your pharmacy, and have them request a refill from us on your behalf. You may also request a refill through our patient portal. Lastly, you can call our office and leave a message on our refill line. In all cases, we request 24 hours to respond to your request.
I need to get in touch with a provider
For urgent matters, please call (623) 544-8400. We are on call 24 hours a day.
For non-urgent matters, our patient portal is the best way to communicate with our medical team during and after business hours. Dr. Stalberg and the PA’s check messages regularly, and generally respond within 24 hours.
If you do not have access to the Patient Portal, you are welcome to leave a message with the medical assistant, who will pass the message to Dr. Stalberg.
I need to see a specialist
To request a referral, please call our office or send us a message through the patient portal. We know many excellent specialists in the area, and we can help connect you with the right practice to meet your needs. Many insurances require a visit with your PCP before seeing a specialist – please check the requirements of your particular plan.
I have a question about my bill
Our practice makes every effort to resolve any issues with the insurance company before sending our your bill. Most questions are best addressed by your insurance company, however you may call us at (623) 544-8400, or
I’d like to request a copy of my medical records
The easiest way to request a copy of your medical records is by sending a secure message through the patient portal.
As an alternative, you can also call our office to make your request. Please allow up to a week in order to process your medical records request. Due to the time and labor involved, our practice charges $5.00 for the first 10 pages, and 25 cents for each additional page. If you are transferring care to a new physician, or the records are needed by a specialist, their office can request the records free of charge.
Insurance and Financial Policies
We are committed to providing you and your family with quality, cost effective care, and we will automatically bill your primary insurance. We accept assignment of all Medicare patients. If you furnish us the information, we will also bill your secondary or supplemental insurance.
It is very important for you to bring your current insurance card(s) each visit and to inform us if there has been any change in your coverage or in your address. As changes may occur in the plans that we accept, please verify our acceptance at the time you make an appointment. Call (623) 544-8400 for more information.
Benefits and eligibility
Although we will bill your insurance, full payment is the patient’s responsibility. We will make every effort to verify your coverage prior to your appointment, yet we cannot guarantee coverage of services by your insurance company. To determine your insurance benefits and eligibility please contact your insurance carrier directly. Note that you are ultimately responsible for payment for all services that are not covered by your insurance plan. Any patient balance after insurance is normally due within 30 days but we are able to set up a payment contract for outstanding accounts. We request payment or co-payment for office services at the time they are rendered unless prior arrangements have been made.
Self Pay Patients
If you are a self-pay patient (if you plan to pay without the use of insurance) we will work with you to determine an appropriate payment schedule.
